The undersigned candidate has requested the Company to help the Candidate find new employment. The Company will not charge a fee to the Candidate for any services. The Company, in performing its services, may seek information about the Candidate's experience and background ("information"). The information may include materials, records, and written and verbal reports relating to the Candidate's prior job experience and performance, education, and criminal record, if any. The information may also include results of tests and assessments administered by the Company and others.
The candidate, by electronically signing this form, authorizes the company to obtain all such information which the company believes appropriate and authorizes the company to provide all such information to prospective employers.
We will NOT contact your current employer until you have resigned.

By electronically signing this Authorization I acknowledge that I have read and received a copy of this Candidate Authorization and Release and that I understand and agree to all of its terms.
I release your company and its employees and agents from all liability relating to your obtaining and use of such information and your providing such information to prospective employers. I authorize all companies, schools, organizations and other persons to provide such information to you and release them from all liability for doing so.
